Acupuncture for Acute Musculoskeletal Pain in the Emergency Department

Acute musculoskeletal pain in the neck, back, and extremities accounts for a substantial percentage of emergency department visits, yet conventional treatment regimens heavily rely on NSAIDs and opioids that carry significant side effects and safety concerns. In this column from the Society for Acupuncture Research, Sandro Graca breaks down a landmark Duke University pragmatic clinical trial evaluating acupuncture integrated into acute emergency care and continued in outpatient clinics. The study examined Battlefield Auricular Acupuncture alongside individualized peripheral needle protocols compared to standard emergency care alone. Although average one-month pain score reductions between groups showed only modest differences, patient satisfaction was dramatically higher among acupuncture recipients, accompanied by an outstanding safety profile. Most crucially, the trial revealed a powerful dose-response relationship: patients completing six or more acupuncture sessions experienced an impressive 66.5% pain reduction, compared to only 44.2% for those with fewer visits. Unfortunately, logistical barriers prevented many emergency patients from attending outpatient appointments. Graca emphasizes that integrating acupuncture successfully into emergency medicine depends on establishing accessible group clinics, streamlined referral pathways, and appropriate dosing schedules, demonstrating that acupuncture is a viable, non-opioid solution for acute musculoskeletal emergencies when adequate follow-up is achieved in collaborative hospital and clinical healthcare settings today.
