How the 1991 LAX collision changed aviation

A major runway collision in 1991 at a busy international airport resulted in a devastating fire and multiple fatalities when a landing passenger jet was cleared onto an active runway already occupied by a smaller commuter aircraft.
The primary cause was traced to an air traffic control error, where the incoming aircraft was mistakenly authorized to land on the wrong runway. At the same time, several safety layers failed to prevent the conflict from escalating. The control tower had limited visibility of the runway environment, and ground surveillance systems were not functioning effectively. The smaller aircraft on the runway was also difficult to distinguish due to lighting conditions that blended into the surrounding environment.
When the larger aircraft made its final approach, neither the crew nor the controller had a complete understanding of the imminent danger until it was too late. The resulting collision led to a severe post-impact fire and significant loss of life.
Investigations identified a combination of human error, inadequate ground monitoring systems, and visibility limitations as key contributing factors. In response, major safety reforms were introduced, including improved runway lighting standards, upgraded ground radar systems, and enhanced procedures for runway occupancy verification. These changes significantly strengthened runway safety and reduced the risk of similar collisions in the future.
