Why Your Loved One Might Be Depressed (Even If They Don’t Seem Sad)

Depression in older adults often doesn't look like sadness at all. Clinicians call it "depression without sadness" — no tears, no visible sorrow, just a flat, numb, nothing feeling. And because it doesn't look like depression, it's one of the most under-recognized health issues in adults over 55.

In this episode, health psychologist Dr. Mary Catherine George gets personally honest — including about being on medication herself — and walks through the three quiet signs of late-life depression that are easiest to miss:

1️⃣ NO ANTICIPATION — You book the trip, schedule the event… and feel nothing. The clinical word is anhedonia: the part of your brain that registers pleasure goes quiet. You catch yourself performing excitement instead of feeling it.

2️⃣ THE SHRINKING WORLD — It creeps up slowly: friends move away, you hesitate to say yes, and staying home always feels easier. The question that reveals it: "When's the last time I said yes to something because I WANTED to?"

3️⃣ BODY COMPLAINTS WITHOUT A CAUSE — Fatigue, achiness, dull headaches, sleep that's never enough — but the doctor says your tests look fine. Late-life depression often shows up in the body, not in tears. "I'm just getting older" is not always the real explanation.

The question that ties it all together: Are you sad — or do you just not feel anything? Sadness is loud; it announces itself. Flatness is quiet, and "at least I'm not crying" is not the same as fine.

MC also shares what actually helps: saying the truth out loud to one trusted person ("I haven't felt like myself lately — not sad, just nothing"), scheduling things anyway, small steps instead of white-knuckling it alone, and the surprising power of doing something in service of someone else.
